Transport Books Order Form

Ordering is quick, convenient & easy! Highlight & copy the entire title, description & price of the item from the catalogue list into the Title space below. Complete the rest of the form. This form is not encrypted in transmission. Until a "secure" online order system is installed, you may wish to transmit your order by any of the following methods, but phone or fax your credit card information. Click on "Submit" to e-mail your order. To add titles to the same order, repeat the procedure, submitting each one separately. (Your choice and your name are sufficient on subsequent forms of the same order.) To fax, phone or post your order print out the completed form. Write in additional choices & fax it to (416)744-7696 anytime, or post it to our address below. You can phone toll free (800)665-2665 during our business hours (Eastern Standard Time) with the form as a reference.

Orders must be prepaid by credit card (Mastercard or Visa upon shipping), cheque or money order (Canadian dollars) payable to: TRANSPORT BOOKS. No COD. Do not mail cash. Library, School & Government purchase orders are invoiced Net 30 days .

Sales Tax: All Canadian shipping addresses add 6% GST on the sub-total of goods + shipping. Ontario shipping addresses also add 8% PST to videos & software only, and NOT to books or GST. E-mail us if you have questions.

Books may be returned within 30 days of receipt for a refund. Videos & software may not be returned. Please see "Returns" below, and contact us with any questions.

Shipping: Goods in stock are shipped when ordered. Back orders & special orders are assembled & shipped as available. Advise us of special requirements in the "message" box below.
anada - Insured Parcel Post is $7.95 + GST per order.
U.S.A. - Insured Parcel Post is $11.95 (Canadian) per shipment.
Overseas - Parcel Post is available but at customer's risk if Postal Insurance is not available. Inquire.
Returns: Books may be returned within 30 days of receipt for a refund, except for "special order" books which together with videos & software, are not returnable. Books must be returned freight prepaid & in original condition with a copy of our invoice. (Shrink wrapping need not be replaced but adequate wrapping & packaging is essential & insurance is advised.) Shipping & handling charges are not refunded. Any defective item may be returned for replacement (only) at no additional cost: Return freight prepaid, packaged as it was sent & with the defect described on a copy of the invoice.
